Hospice and Palliative Care
The Hospice program at Holy Name Medical Center provides individualized and compassionate care for terminal patients whose illness is no longer responsive to cure-oriented treatments. The care provided is designed to not only provide pain relief and management of physical symptoms but emotional and spiritual support as well. This concept of caring helps patients - and their loved ones - live each day to the fullest while enhancing comfort and quality of life all within the comforts of the caregiver or patient's home. Additionally, supports services are available to the patient's family, caregivers and loved ones during illness and into bereavement.
The core of Hospice is a cooperative effort by family, friends, and a team of professional and volunteer caregivers working together to meet the multiple needs of the patient. The role of the family or caregiver is vital to the foundation of hospice. The primary caregiver, typically a family member or someone close to the patient, assumes the responsibility for the patient's care with the guidance and support of the hospice team members.
The Hospice team includes highly skilled and trained professionals, including: the medical director, visiting registered nurses, the social worker, the inter-faith clergy, the bereavement counselor, home health aids and volunteers. Through their collective efforts, the Hospice team provides clinical expertise and respite for the primary caregivers.
The Hospice team will meet regularly to help ensure that the needs of the patient and caregivers are being considered fully. The patient's primary physician is an integral part of the team and will remain responsible of the patient's general medical care for the duration of hospice services.
