Pfizer Pediatric Vaccine (5-11 years): The CDC and NJ Department of Health have approved the pediatric formulation of COVID-19 vaccine for children 5 to 11 years of age. Holy Name is now administering the Pfizer/BioNTech COVID-19 Vaccine to 5-11 year olds. Parents can now schedule appointments for their children 5 years and older at Holy Name's Hackensack/Teaneck site. A parent must be present and sign consent for their minor child.

Booster Doses:

• Pfizer & Moderna: ALL adults aged 18 and older who received the second dose of their 2-dose primary series at least 6 months ago.
• J&J (Janssen): ALL adults aged 18 and older who received a dose of J&J at least 2 months ago.
• Mix & Match Booster Doses: Eligible people may choose which vaccine they receive as a booster dose. We encourage you to speak to your doctor if you have questions.

For the Immunocompromised: People with severely weakened immune systems are approved to receive a 3rd dose of the same vaccine at least 28 days after completing two doses of Pfizer or Moderna. For more information visit CDC.gov/immuno.

Eligibility: Everyone who schedules an appointment should first determine if they are eligible and will sign the online form stating that they are eligible and it is the correct time for their booster. For more information on booster eligibility please visit CDC.gov.

Vaccine availability:

• Pfizer Vaccine: ALL Holy Name sites offer Pfizer vaccine for all those who are eligible.
• J&J and Moderna Vaccine: Offered ONLY at our Teaneck/Hackensack site, now located at 3 University Plaza, Hackensack.
• Pfizer Pediatric Vaccine (5-11 years): Offered ONLY at the Teaneck/Hackensack location. Registration is now open for this age group.
